Job Opportunity: Advanced Clinical Practitioner

An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Mearns cluster. We are seeking an exceptional individual to help us deliver excellent care in Aberdeenshire.

Role Overview:

The Advanced Clinical Practitioner (ACP) role is diverse and autonomous. You will provide enhanced clinical assessment and treatment to meet community patient needs, support medical staff and healthcare professionals, and collaborate with independent practices to ensure high-quality care across clusters. Responsibilities include acute triage and home visits as needed.

Position Details:

• Work across multiple clinical settings including GP surgeries, community hospitals, and patients' homes.
• Must be an experienced registered professional with evidence of advancing practice.
• Understand conditions requiring hospital admission or virtual community ward management.
• Manage patients at home as part of a multidisciplinary team.
• Be proactive in developing the ACP role within Aberdeenshire.

Qualifications and Requirements:

• Adherence to The Advanced Care Academy framework, including a portfolio (mandatory completion within an agreed timeframe).
• Consideration may be given to candidates demonstrating a commitment to advanced practice and academic progression.
• Flexible working hours; part-time considered.
• Valid driving license and access to a vehicle; willingness to travel within the Mearns cluster.
