Job summary

Birmingham Community Healthcare NHS Foundation Trust is seeking a Bank Band 7 Single Point of Access Clinical Practitioner for night shifts. Based at Priestley Wharf, you will provide advanced triage, assessment and care planning for patients with acute or undiagnosed conditions, to prevent admissions and support timely discharge.

You will work within a seven‑day service, typically on shifts from 20:00 to 08:00, with flexible bank hours to meet service needs.

Qualifications

• Registered healthcare professional eligible to work in the UK.
• Flexible bank position covering a seven‑day service.
• Night shift pattern with potential shifts 20:00‑08:00.

Responsibilities

• Provide advanced clinical triage and assessment for acute/undiagnosed conditions.
• Prioritise referrals and manage clinical risk.
• Prescribe independently and review medications as part of patient care.
• Maintain accurate clinical records and liaise with multidisciplinary teams.
• Support admission avoidance and safe discharge planning.
