Job summary

Birmingham Community Healthcare NHS Foundation Trust is seeking a Band 7 Night Bank Single Point of Access Clinical Practitioner to provide advanced clinical triage, assessment and decision-making for patients with acute, complex or undiagnosed needs.

You will prioritise referrals, initiate investigations, interpret results, prescribe where appropriate and coordinate safe discharge or transfer of care, while leading and supporting colleagues and contributing to service planning and quality

Responsibilities

• Provide advanced clinical triage, assessment and decision-making for patients with acute, complex or undiagnosed needs.
• Prioritise referrals, manage clinical risk and respond appropriately to urgent or emergency situations.
• Initiate investigations, interpret results, prescribe medication and refer patients to appropriate services.
• Work collaboratively with patients, carers and multidisciplinary teams to develop, monitor and review individual care plans.
• Support admission avoidance and coordinate safe discharge or transfer of care.
• Provide senior clinical leadership, supervision, training and support to colleagues.
• Contribute to service planning, audit, quality improvement and the implementation of clinical policies and pathways.
