Job summary

Birmingham Community Healthcare NHS foundation Trust is seeking a Band 7 Bank Clinical Practitioner to provide advanced triage, assessment and clinical decision-making for patients with acute or complex needs. You will prioritise referrals, manage risk and respond to urgent situations.

You will initiate investigations, interpret results, and refer to services, while supporting discharge planning and providing senior clinical leadership, supervision and training to colleagues.

Qualifications

• Provide advanced clinical triage, assessment and decision-making for patients with acute, complex or undiagnosed needs.
• Prioritise referrals, manage clinical risk and respond to urgent or emergency situations.
• Initiate investigations, interpret results, prescribe medication and refer patients to appropriate services.
• Work with patients, carers and multidisciplinary teams to develop and monitor individual care plans.
• Support admission avoidance and coordinate safe discharge or transfer of care.
• Provide senior clinical leadership, supervision, training and support to colleagues.
• Maintain accurate clinical records and comply with safeguarding, clinical governance, confidentiality and infection-control requirements.
• Contribute to service planning, audit, quality improvement and the implementation of clinical policies and pathways.
