Job summary

Lincolnshire County Council's Hospital Social Work Team at Lincoln County Hospital is looking for a Level 1 Social Worker to join a busy, supportive hospital environment. The role offers 37 hours per week on a permanent basis, with opportunities to work across multiple sites in Northern Lincolnshire and Goole hospitals.

You will develop skills in safeguarding, care act assessments and discharge planning, supported by experienced colleagues, with regular supervision and a strong team culture

Responsibilities

• Manage a caseload with supervision, developing your professional confidence and autonomy
• Undertake Care Act 2014 assessments, care and support planning, and reviews
• Work collaboratively with individuals, families, carers and professionals to identify strengths
• Contribute to safeguarding enquiries, ensuring the person’s voice and desired outcomes remain central
• Work closely with health colleagues to support safe and timely hospital discharges
• Identify wider needs such as housing, health or community support and work with partners to address them
• Promote strengths-based practice and support people to build and maintain support networks
