Job summary

Salford City Council’s Neuro and Stroke Social Work Discharge Team is recruiting a Part-Time Social Worker (18 hours) for acute hospital discharge planning. You will assess, plan, and coordinate with multidisciplinary colleagues to support safe, timely discharges and promote independence for adults with complex needs.

You will work across wards and clinics, safeguarding and recording information while adopting a strengths-based, person-centred approach to maximise patient outcomes.

Qualifications

• Qualified Social Worker with current registration and experience in acute hospital settings.
• Ability to conduct person-centred, holistic assessments and care planning.
• DBS check is a requirement for this role.

Responsibilities

• Provide a person-centred statutory social work service for vulnerable adults and carers.
• Support integrated, multidisciplinary working and timely discharges.
• Safely manage risk, record and share information with partners.
• Use strength-based approaches to meet individual needs and support planning.
• Contribute to team development across wards and clinics.
