Job summary

Salford Council is seeking an experienced Social Worker to join the Neuro and Stroke Social Work Discharge Team at Salford Royal Hospital. You will assess, plan and support discharge to home, coordinating with medical, nursing, therapy and reablement colleagues to promote independent living.

You will uphold person-centred, strengths-based practice, manage risk and contribute to team development across wards and clinics, including care for patients with complex needs such as MND.

Qualifications

Job Summary

An exciting opportunity has arisen for an experienced Social Worker to join the Neuro and Stroke Social Work Discharge Team at Salford Royal Hospital.

Working within a fast-paced acute hospital setting, you will play a key role in supporting patients to return to their communities and live as independently as possible. You will work closely with multidisciplinary colleagues, including medical, nursing, therapy, and reablement teams, all focused on delivering personalised, holistic assessments and achieving safe, timely discharges.

We are seeking a motivated and experienced practitioner with excellent assessment, safeguarding, communication, and partnership-working skills. You will be committed to delivering person-centred care, promoting independence, and supporting individuals to achieve positive outcomes following hospital admission.

• To provide a person centred, statutory social work service for the most vulnerable adults and their carers in accordance with local and national policies and procedures.
• To actively support and participate in the development and implementation of integrated, multi-disciplinary working.
• To take a key role working with partners to safeguard service users through the effective management of risk and the recording and sharing of information.
• To adopt a strength's-based approach to meet the individual needs of people with lived experience through effective support planning.
• To contribute to the effective development of the team.
• To work across various wards and clinics, working with a client group whose needs can be complex and challenging but very rewarding. The clinic work is Greater Manchester wide and provides invaluable support to people diagnosed with MND for example which is very beneficial especially for those at the early stages of such a diagnosis.
