Job summary

Salford Royal Hospital is seeking an experienced Social Worker to join the Neuro and Stroke Social Work Discharge Team on a part-time basis (18 hours). Working within a fast-paced acute hospital, you will support patients to return to their communities and live as independently as possible, collaborating with medical, nursing, therapy and reablement colleagues.

Responsibilities

• Provide a person-centred, statutory social work service for the most vulnerable adults and their carers in accordance with local and national policies and procedures.
• Actively support and participate in the development and implementation of integrated, multi-disciplinary working.
• Take a key role working with partners to safeguard service users through the effective management of risk and the recording and sharing of information.
• Adopt a strength’s-based approach to meet the individual needs of people with lived experience through effective support planning.
• Contribute to the effective development of the team.
• Work across various wards and clinics, working with a client group whose needs can be complex and challenging but very rewarding; clinic work is Greater Manchester wide and supports people diagnosed with conditions such as MND, especially in early stages.
