Job summary

Salford Council is seeking a qualified Social Worker to join the Neuro and Stroke Social Work Discharge Team at Salford Royal Hospital on a part-time basis (18 hours per week). In this fast-paced acute hospital setting you will support patients to return home and live as independently as possible, working with medical, nursing and therapies to enable safe, timely discharges.

You will be an experienced practitioner with excellent assessment, safeguarding, communication and partnership-working

Qualifications

• Experience as a Social Worker in acute hospital settings preferred.
• Ability to work with multidisciplinary teams and fast-paced environments.
• Strong assessment and safeguarding skills.

Responsibilities

• Provide a person-centred statutory social work service for vulnerable adults and carers.
• Support integrated multi-disciplinary working and discharge planning.
• Safeguard service users through effective risk management and information sharing.
• Use a strengths-based approach to meet individual needs and develop support plans.
• Contribute to team development and service improvements.
• Work across wards and clinics, with complex but rewarding caseloads including conditions like MND.
