You will track delivery, manage actions, risks, and timelines, and translate neighbourhood priorities into practical service improvements, strengthening links between general practice, PCN services, and local partners. This role requires strong organisation and stakeholder engagement skills.

Responsabilités

• Coordinate PCN neighbourhood actions and delivery workstreams across member practices and partners.
• Track actions, risks, and timelines; ensure timely delivery and escalation where needed.
• Support translation of neighbourhood priorities into practical actions for general practice and PCN services.
• Strengthen operational links between general practice, PCN services, and wider neighbourhood partners.
• Prepare reports, briefings and presentations for governance and stakeholder briefings.
