Job summary

Responsibilities

• Lead the development and implementation of the Neighbourhood Health Delivery Model, aligned with strategic and local population health priorities.
• Lead commissioning approaches for multidisciplinary neighbourhood teams, including specifications, market management and procurement.
• Translate strategic ambitions into operational delivery, supporting integrated, preventative and person-centred models of care.
• Lead co-production, review and monitoring of the Better Care Fund with local authorities and partners.
• Support oversight of joint funding arrangements (Section 75/76) and delegation agreements, ensuring governance.
• Build relationships across NHS providers, local authorities, primary care, neighbourhood teams and VCSEF.
• Support performance management, assurance and improvement, monitoring outcomes and risks.
• Use data-driven commissioning to improve health outcomes and reduce inequalities.
• Manage budgets and resources, identify efficiency opportunities and value for money.
• Provide senior support for patient flow and UEC demand, including winter planning.
• Maintain governance and compliance; contribute to policies and provide reports to leadership.
• Provide visible leadership and manage teams; foster development and continuous improvement.
• Represent the neighbourhood agenda at system forums, boards and partnerships.
