Job summary

Moorlands Surgery, within Darlington Primary Care Network, seeks an ARRS-funded coordinator to lead the operational delivery of neighbourhood workstreams and manage PCN-funded services. You will track improvements and escalate issues within delegated authority, reporting to the PCN Business and Governance Manager.

The role focuses on coordinating actions, collating data, and preparing reports for diverse stakeholders, including member practices and partner organisations.

Qualifications

• Relevant qualification or equivalent experience in care coordination, project or programme support.
• Evidence of continuing professional development and willingness to train further.
• Coordinating activities or projects involving different teams, services or stakeholders.
• Following through agreed actions to agreed timescales.
• Identifying operational issues and contributing to service improvements.
• Managing a varied workload and competing priorities.
• Producing clear, accurate written reports, records or updates.
• Handling confidential or sensitive information appropriately.
• Nice to have PM/quality or data analysis qualifications.

Responsibilities

• Coordinate neighbourhood and Integrated Neighbourhood Team workstreams.
• Coordinate reviews of PCN-funded and commissioned services.
• Track service improvements to completion and raise delays, risks and problems promptly.
• Develop and maintain project documentation, including milestones, risks, dependencies and outcome measures.
• Act as the operational link between the PCN and member practices on agreed work.
• Support engagement between member practices and partner organisations.
• Collate and analyse activity, demand and outcome information, checking data quality and identifying trends.
• Support evaluation of implemented changes and capture learning for future service development.
• Facilitate meetings, workshops or task-and-finish groups and ensure actions are clear.
• Attend operational meetings and delivery groups on behalf of the PCN where delegated.
• Escalate strategic, contractual, financial, workforce or governance matters to the appropriate arrangements.
• Produce clear reports, briefings and presentations for different audiences.
