The Neighbourhood Connector will act as a single point of contact for patients and their representatives. They will develop relationships that will enable them to work in a personalised way to help the person live their best life possible.
The post holder will support a defined group of individuals that will benefit from a case management/self-care approach to promote independence. To visit individuals at home to undertake personal assessments and ensure that every person is supported to live the life they wish, to the best of their abilities.
The Neighbourhood Connector will need to e stablish and maintain excellent communication with individuals and explore complex issues relating to care options and decisions and to sustain effective working relationships across all health and social care services.

They will have the skills and that will enable them to support the person to navigate services and activities that are available to them in their community and make informed choices.
