Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a Registered Nurse (Adult) to join the new Home First service in Stroud. The role focuses on timely assessments, personalised care plans and safe hospital discharge for patients returning home.

You will work within a multidisciplinary team, delivering high-quality, patient-centred care in patients' homes and communities, with autonomy and strong clinical leadership to support ongoing independence and positive outcomes.

Qualifications

  • Registered Nurse (Adult / RGN)
  • Current NMC registration
  • Degree in health related subject or working towards this with 50%+ Level 3/6 credits
  • Individual modules at degree or masters level, e.g. PACR
  • Recognised teaching/mentoring qualification

Responsibilities

  • Complete holistic nursing assessments and develop personalised care plans for patients discharged through Home First and other discharge-to-assess pathways
  • Support safe and timely hospital discharges and help prevent avoidable hospital admissions
  • Deliver evidence-based nursing care, treatment, and intervention for patients with complex conditions
  • Identify ongoing nursing needs and coordinate handover to Community Nursing Teams
  • Work with therapists, reablement staff, GPs, social care teams and system partners across One Gloucestershire
  • Promote independence through health coaching, education and goal-setting
  • Support medicines management and clinical decision-making
  • Provide advice to patients, carers and families to maximise health and self-management
  • Contribute to service improvement, quality assurance, audit activity and patient safety initiatives
  • Support development of colleagues through supervision, coaching and leadership
  • Manage and prioritise a dynamic caseload with good clinical judgment

Are you a passionate and motivated Registered Nurse looking to make a real difference to patient outcomes? This is an exciting opportunity to join ournew Home First service that launched on 1 September, supporting patients to return home safely from hospital and avoid unnecessary admissions.

Working as part of a multidisciplinary team, you will deliver timely, holistic assessments and develop personalised care plans for patients with complex health needs. You will play a key role in supporting hospital discharge, promoting independence, and ensuring patients receive the right care, at the right time, in the right place.

Main duties of the job

  • Promote independence through health coaching, education, goal-setting, and personalised care approaches.
  • Support medicines management and clinical decision-making to optimise patient outcomes.
  • Provide advice and support to patients, carers, and families to maximise health, wellbeing, and self-management.
  • Contribute to service improvement, quality assurance, audit activity, and patient safety initiatives.
  • Support the development of colleagues and students through supervision, coaching, and leadership within the team.
  • Manage and prioritise a dynamic caseload, demonstrating excellent clinical judgement and organisational skills.

QUALIFICATIONS
  • Registered Nurse (Adult / RGN)
  • Current NMC registration
  • Holds degree in health related subject or be working towards this, with over 50% of level 3 or 6 credit already obtained
  • Individual modules at degree or masters level, eg. PACR
  • Holds recognised teaching/ mentoring qualification
LENGTH AND / OR NATURE OF EXPERIENCE
  • Experience of working without direct supervision
  • Consolidated experience of community nursing practice
  • Experience of managing long term conditions and case management of complex patients
  • Experience of delivering extended role clinical interventions
  • Evidence of post-registration education & training.
PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE
  • Ability to work on own initiative, assess patients competently and link with colleagues in the wider MDT to tailor plans of care
