Job summary

Midlands Partnership NHS Foundation Trust is recruiting an NMC Registered Community Staff Nurse to join our busy Community Nursing Team based at Merlin House. You will assess, plan and deliver nursing care to patients in the community, travel across the patch as required and work within evidence-based practice.

This role is suitable for newly qualified nurses, offers induction, ongoing training, and opportunities for career development.

Qualifications

• Degree in Nursing or equivalent.
• Registration with NMC.
• UK Registration required.

Responsibilities

• Promote independence and support patient centred care planning.
• Provide care directly to patients on the caseload with a range of nursing needs.
• Work autonomously, in line with your scope of practice, competence and policies.
• Handover patient information to the wider team, document patient information effectively.
• Escalation of patients needs as appropriate.
