Nottinghamshire Healthcare NHS Foundation Trust in Retford is seeking a Band 3 Health Care Assistant to join the Urgent Community Response team. You will support patients to regain independence, prevent admission and deliver person-centred care, rehabilitation and reablement.

This mobile, community-based post requires travel across a large county, visiting patients in their homes and using electronic records.

Qualifications

• NVQ Level 3 qualification (or equivalent) in a relevant health & social care area.
• Relevant clinical skills.
• Competency based education package or prepared to undertake

Responsabilités

• Assist patients with personal, physical and emotional needs.
• Deliver rehabilitation and care with the aim of improving function.
• Collect data to evaluate clinical interventions.
• Support timely discharge and maintain independence at home.
• Work with nurses, therapists and the multidisciplinary team
