Nottinghamshire Healthcare NHS Foundation Trust is seeking a Band 3 Health Care Assistant to join the Urgent Community Response team in Bassetlaw. The role involves mobile, community-based visits across a large county to support patients to regain independence and prevent admissions.

You will work under the direction of a registered professional, providing rehabilitation, care planning, and data collection to monitor outcomes. A full UK driving licence and vehicle for business use are required.

Qualifications

• The post holder will assist patients with their personal, physical and emotional needs, within the overall programme of care as directed by the registered professional.
• To undertake nursing, therapy and social support activities following training and demonstration of competency, delegated by the relevant profession.
• To work without direct supervision undertaking tasks delegated by the relevant profession in order to support the team.
• To deliver high quality care to patients within the home and clinic setting, respecting patients.
• To ensure that consent is obtained before any intervention is implemented.
• Respect the patient and carers as individuals always respecting privacy and dignity.
• Participate in administering medication, hard collar care, stoma care and eye drops.
• Provides advice which contributes to the care, wellbeing or education of patients, including Health Promotion.
• Maintain confidentiality of all information gained during course of duty.
• Work flexibly to meet individual health care needs of the patient and carer.
