Job summary

Hywel Dda University Health Board is seeking a Part-time Community Mental Health Centre Support Worker (Band 3) to join our community team in Haverfordwest. The role involves delivering compassionate care to individuals with moderate to severe mental health needs, working within a multidisciplinary team, and supporting recovery journeys in community settings.

The post is 22.5 hours per week (09:00 - 17:00) with a pro rata salary of £26,813–£27,890 per annum.

Qualifications

• Experience of working in a relevant health or social care environment.
• Ability to work as part of a multidisciplinary team.
• Commitment to compassionate, patient-centred care.

Responsibilities

• Support senior practitioners to deliver responsive care for a caseload of individuals with mental health difficulties.
• Provide care in patients' homes, in the community and in health settings.
• Record contacts in clinical notes promptly and assist with admin tasks.
• Support holistic approaches to recovery and maintain accurate documentation.
• Collaborate with Welsh-speaking and English-speaking colleagues as needed.
