P&S Care & Support is recruiting a Mental Health Support Worker for community-based services in Basildon. You will support adults with enduring mental health needs to build independence and confidence, helping them work towards recovery in their own homes and local settings.

The role involves practical, social and emotional support, coordinating with external agencies, maintaining records and contributing to care plans.

Qualifikationen

• Experience working with adults with mental health needs or in similar support.
• Understanding of recovery-focused support and independence goals.
• Strong communication and interpersonal skills.
• Flexible, patient and able to work independently or in a team.
• Awareness of housing-related issues and dual diagnosis benefits.

Aufgaben

• Provide practical, social and emotional support in homes and community.
• Help access services, maintain accommodation and develop living skills.
• Collaborate with external agencies to ensure appropriate support.
• Maintain records, contribute to plans and risk assessments.
• Encourage participation in community activities, education, training and employment.
• Work across services/locations as needed.
