Responsibilities:

• Act as an allocated social worker, responsible for delivering statutory social work services to individuals requiring specialist mental health services.
• Collaborate with the mental health hospital discharge hub to facilitate Care Act assessments, s117 aftercare planning, safeguarding, and other statutory functions.
• Manage a diverse caseload, working with individuals, families, and carers to conduct assessments and reviews.
• Work across a small county-wide team, ensuring seamless discharge processes and liaising with mental health hospitals.
• Uphold professional standards, ensuring compliance with legislation and high-quality service delivery.
