Mental Health Social Worker – Hospital Discharge Team – Chichester, West Sussex (UK) – £35 per hour – Part-time 18.5 hours – 3 months

What you will do:

• Act as an allocated social worker, responsible for delivering statutory social work services to individuals requiring specialist mental health services.
• Collaborate with the mental health hospital discharge hub to facilitate Care Act assessments, s117 aftercare planning, safeguarding, and other necessary statutory functions.
• Manage a diverse caseload efficiently, working alongside individuals, families, and carers to conduct strengths-based assessments and reviews.
• Work across a small county-wide team, ensuring seamless discharge processes for patients liaising with mental health hospitals strategically.
• Uphold professional standards and practices, ensuring compliance with legislation while maintaining high-quality service delivery.
