Job summary

Delta Wellbeing Ltd in Carmarthen is seeking a registered Social Worker to join the Community Mental Health Team on a fixed-term basis until July 2027. The role involves managing a caseload of adults with mental health needs, coordinating care, and delivering person-centred interventions within a supportive multidisciplinary team.

Applicants should hold a Degree in Social Work (or DipSW/CQSW), be willing to obtain an enhanced DBS, and have experience or interest in adult mental health services.

Qualifications

• Must be registered as a Social Worker with an appropriate degree or qualification.
• Enhanced DBS with Update Service required or willingness to obtain.
• Experience with individuals experiencing mental health difficulties is advantageous.

Responsibilities

• Manage a caseload of individuals with severe mental health needs.
• Act as Care Coordinator under relevant mental health legislation.
• Undertake assessments and interventions under social care acts.
• Collaborate with families, carers and partner agencies to promote recovery.
• Deliver person-centred interventions guided by co-production principles.
