Wokingham Borough Council is seeking a Social Care Practitioner to join the Integrated Mental Health Team in Adult Social Care on a full-time, permanent basis. You will work with experienced professionals to support adults with mental health needs, focusing on what matters to each person and helping them stay safe and connected to their community.

You’ll progress with supervision, with a clear development pathway toward Grade 6 after meeting progression criteria.

Formación

• Experience in health, social care or a customer-facing role.
• GCSE Maths and English, or equivalent.
• Good communication and IT skills.
• Understanding of safeguarding and confidentiality.
• Ability to stay calm under pressure.
• Commitment to helping people stay independent.
• Experience of supporting vulnerable adults or working in an Occupational Therapy setting.

Responsabilidades

• Get to know people and understand what matters to them.
• Complete assessments and support plans that help people stay safe and independent.
• Give advice and help people find the right support.
• Spot safeguarding concerns and work with others to keep people safe.
• Work closely with families, health teams and other services.
• Keep clear records and support registered practitioners with more complex cases.
