Job summary

Wokingham Borough Council is seeking a Social Care Practitioner to join the Integrated Mental Health Team in Adult Social Care on a full-time, permanent basis. You'll work alongside experienced social care professionals, supporting adults with serious or suspected mental health conditions and their carers.

Under supervision, you’ll assess needs, develop support plans, spot safeguarding concerns, and help people stay safe, independent and connected to their community.

Qualifications

• Experience in health, social care or a customer-facing role.
• GCSE Maths and English, or equivalent.
• Good communication and IT skills.
• Understanding of safeguarding and confidentiality.
• Ability to stay calm under pressure.
• Commitment to helping people stay independent.
• Experience of supporting vulnerable adults or working in an OT setting.

Responsibilities

• Manage own cases with support and guidance from experienced practitioners.
• Get to know people and understand what matters to them.
• Complete assessments and support plans to help people stay safe and independent.
• Give advice and help people find the right support.
• Spot safeguarding concerns and work with others to keep people safe.
• Work closely with families, health teams and other services.
• Keep clear records and support registered practitioners with more complex cases.
