Wokingham Borough Council is seeking a Social Care Practitioner to join the Integrated Mental Health Team in Adult Social Care on a full-time, permanent basis. You will support adults with mental health needs, work under supervision, and help people stay safe, independent and connected to their communities.
You will build relationships with service users and carers, complete assessments and support plans, provide practical advice, and collaborate with families, health teams and other services to
Qualificações
- Experience in health, social care or a customer-facing role.
- GCSE Maths and English, or equivalent.
- Good communication and IT skills.
- Understanding of safeguarding and confidentiality.
- Ability to stay calm under pressure.
- Commitment to helping people stay independent.
- Experience supporting vulnerable adults or working in an Occupational Therapy setting.
Responsabilidades
- Manage own cases with guidance from experienced practitioners.
- Get to know people and understand what matters to them.
- Complete assessments and support plans to keep people safe and independent.
- Give advice and help people find the right support.
- Spot safeguarding concerns and work with others to keep people safe.
- Work closely with families, health teams and other services.
- Keep clear records and support registered practitioners with complex cases.
