K & W Healthcare is seeking a Mental Health Practitioner to join a PCN multidisciplinary team. The post holder will assess, support and manage patients with mild to moderate mental health issues, delivering brief interventions and appropriate onward referrals.

Qualifikationen

• Registered RMN with valid NMC registration or equivalent.
• Minimum 2 years post-registration experience in mental health services.
• Experience with mild to moderate mental health conditions.
• Strong communication and interpersonal skills.
• Knowledge of mental health legislation and risk assessment.
• Ability to work independently within a primary care environment.

Aufgaben

• Undertake holistic mental health assessments within a primary care setting using evidence-based frameworks.
• Provide brief interventions for mild to moderate mental health conditions (e.g., anxiety, depression).
• Develop, implement, and review person-centred care plans in collaboration with patients and the MDT.
• Deliver low-intensity psychological interventions and psychoeducation.
• Signpost and refer patients to Talking Therapies (IAPT) or other services as appropriate.
• Maintain accurate clinical documentation in line with standards.
