North Kerrier West Primary Care Network is seeking a qualified Mental Health Practitioner to support our multidisciplinary team in delivering proactive, person-centred mental health care in the community. The post involves collaboration with GPs, social prescribers and secondary care to design and implement effective care pathways.

You will provide clinical expertise, oversee risk management, and contribute to service development and learning within the PCN landscape in Cornwall.

Qualifikationen

• Registered Nurse with current registration (mental health/LD/adult).
• Evidence of ongoing CPD and professional development.
• Non-medical Prescriber is desirable but not required.

Aufgaben

• Provide clinical expertise and support to GPs and practice clinicians to manage patients with mental health problems in primary care.
• Triage and assess patients presenting with mental health concerns; develop care plans and reviews.
• Collaborate with MDTs and external services to deliver integrated care and timely access.
• Support non-medical prescribing where possible and contribute to training of staff.
