Job summary

Primary Care Sheffield is seeking a Mental Health Practitioner to join our PCN team, providing skilled interventions for adults with mental health problems. You will work within a multidisciplinary primary care team, managing your own patient group and coordinating with carers and other professionals to deliver person-centred care.

We value experience in mental health settings, leadership in psycho-social care, and the ability to supervise learners while maintaining high standards of governance

Qualifications

• Experience with mental health care in primary care settings and risk management.
• Evidence of leadership in psycho-social interventions for patients and carers.
• Proven ability to supervise and support team members.

Responsibilities

• Manage your own patient group in line with GP procedures and risk protocols.
• Assess needs, identify problems and plan care with patients and carers.
• Implement and review care plans; take action when required.
• Advise and support patients and carers to promote health and wellbeing.
• Maintain communication with agencies involved in care in hospital and community settings.
• Provide leadership for psycho-social interventions within a multi-disciplinary team.
