Job summary

K&W Healthcare is seeking an Experienced General Practice Nurse to support West PCN across designated GP practices. The role is self-employed with an hourly rate of £20-£30 and up to 37.5 hours per week, aligned to practice requirements.

The post holder will deliver patient-centred nursing care, conduct long-term condition reviews, vaccinations and wound care, while working closely with GPs and the wider team to promote preventive care and safe clinical practice.

Qualifications

• Current NMC registration as a nurse.
• Experience in general practice nursing.
• Competence in vaccinations and preventive care.
• Ability to work across multiple practices within West PCN.
• Strong clinical documentation and patient communication.

Responsibilities

• Conduct nursing assessments and create care plans.
• Deliver reviews for long-term conditions (diabetes, COPD, etc.).
• Administer vaccinations and immunisations.
• Perform wound care, blood pressure checks and venepuncture.
• Collaborate with GPs and practice team to support patient care.
