Job summary

South West Yorkshire Partnership NHS Foundation Trust is recruiting Mental Health Practitioners to strengthen PCN teams across Wakefield. You will deliver bio-psycho-social assessments and interventions within GP surgeries, working with MDT colleagues to embed mental health support in primary care.

The role is based in a PCN in the east of the district; agile working applies and standard hours are 9 to 5, five days a week.

Qualifications

• RMN registered with current NMC registration.
• Experience in community mental health settings.
• Ability to work within multidisciplinary teams and with carers.

Responsibilities

• Conduct mental health assessments in the community.
• Provide leadership to Core CMHT and supervise team members.
• Coordinate care plans and referrals with GP surgeries and MDTs.
• Maintain accurate patient records and governance documentation.
• Contribute to service development and training of staff.
