Job summary

North Kerrier West Primary Care Network seeks an enthusiastic Mental Health Practitioner to join our growing Mental Health & Wellbeing Team in Cornwall. Work alongside GPs and a multidisciplinary group to deliver personalised, proactive mental health care within a forward-thinking PCN.

You will help shape new neighbourhood mental health and wellbeing initiatives, contribute to service development, and be supported in professional development and non-medical prescribing where appropriate.

Responsibilities

• Deliver personalised, proactive and preventative mental health care within primary care teams.
• Collaborate with GPs, Social Prescribers, and wider system partners to coordinate care.
• Contribute to service development, innovation and reducing health inequalities.
