Job summary

Northpoint is seeking a Healthy Minds Practitioner in Leeds for a fixed-term maternity cover (7 months) at 35 hours per week. The role, hybrid with standard hours, offers £32,073 per annum pro rata (Agenda for Change Band 5).

You will triage referrals and support patients to improve wellbeing. Key duties include recording referrals, managing complex cases, maintaining confidentiality, and achieving service activity targets.

Qualifications

• Professional therapeutic qualification or degree.
• Minimum of two years' experience delivering mental health interventions.
• Good understanding of common mental health problems.

Responsibilities

• Accept and record referrals using patient management system (SystemOne/EMIS).
• Identify and manage cases with complex needs, including triage and signposting to appropriate support.
• Manage a full caseload and meet activity levels while keeping treatment accessible.
• Protect patient confidentiality in line with policies.
