Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Band 6 Senior Mental Health Practitioner to support pregnant people and postnatal individuals with moderate to severe mental health needs.

The role is 37.5 hours per week, based in Wokingham with a mix of remote, community and office work. You’ll join a multidisciplinary team and contribute to safe, compassionate, evidence-based care.

Qualifications

• Must be RMN or SW qualified and licensed to assess, plan and evaluate care for patients with mental health problems.
• Experience with multidisciplinary teams and complex perinatal care.
• Understanding of health and social care systems and Mental Health Act.
• Ability to work autonomously within a multidisciplinary environment.
• Commitment to supporting families and recovery in the community.

Responsibilities

• Act as a Care Coordinator delivering trauma-informed, evidence-based interventions to birthing people and families.
• Undertake comprehensive specialist assessments and develop collaborative care plans.
• Provide or transfer into therapeutic interventions that promote recovery and positive parent-infant relationships.
• Collaborate with midwives, health visitors, and social care partners to ensure holistic care.
• Support risk management, safeguarding, and case formulation; liaise with maternity services and primary care.
