Job description

Job Title: Mental Health Practitioner (Band 7) - Step Forward Team

This role will be based in Liverpool.

An exciting opportunity has arisen to join our Step Forward Team as a Mental Health Lead working across the Mental Health Care Division footprint. The position is based at a neighbourhood level, delivering a population-based approach to care focusing on prevention and early intervention for mental health.

We are seeking to recruit one permanent Band 7 Mental Health Practitioner to join the team, lead service development, and provide senior clinical leadership. You will utilize your leadership and clinical skills to ensure seamless, integrated, responsive, and holistic care throughout each service user's journey.

Key Responsibilities:
  1. Focus on the interface between Primary and Secondary care, adopting a collaborative care model to ensure access to rapid, evidence-based clinical input and specialist support when needed.
  2. Work in primary care settings with specific PCNs, Integrated Care Teams, and Community Mental Health Teams/other specialist teams.
  3. Operate autonomously and as part of a team, demonstrating resilience and adaptability.
  4. Develop strong working relationships across Primary and Secondary care, VCSFE, and local networks to facilitate collaborative integrated working.
  5. Provide clinical oversight and leadership, requiring senior clinical and operational management skills and experience.
