Job summary

Catholic Charities Archdiocese of Washington is seeking a bachelor’s-level Community Support Specialist 2 to work with adults with mental illness. You will perform case management within an interdisciplinary team, delivering services across home, clinic, and telehealth settings while documenting care and progress in a timely manner.

Responsibilities include IRP development, crisis support, relapse prevention, and linking clients to community resources.

Qualifications

• Bachelor’s degree in social work, counseling, or related discipline.
• Social Work licensure in the appropriate jurisdiction required.
• One year direct service experience in a social service setting with adults.

Responsibilities

• Deliver community support services to identified adults with mental illness.
• Establish productive relationships to promote IRP goals and update IRPs.
• Provide interventions specified in the IRP to each consumer.
• Support consumers during mental health crises and provide Care Point contact after hospital discharge.
• Develop Crisis Plan strategies to avoid hospitalizations.
• Assist with substance use disorder challenges and relapse prevention planning.
• Help consumers build coping skills for home, school, and work.
• Provide mental health education and support to families and networks.
• Assist with daily living activities and healthcare benefits access.
• Link consumers to community resources and coordinate with the treatment team.
