Job summary

Catholic Charities of the Archdiocese of Washington seeks a Clinical Care Coordinator to assess functioning and develop individualized rehabilitation plans for adults with mental illness, using the Recovery model. You will lead IRP development with the ACT team, coordinate hospital treatment planning and transitions, and educate clients on ADL and health benefits, while adhering to DMH guidelines.

This role requires a Bachelor's in Social Work, a DC license, and at least three years of mental

Qualifications

• Bachelor's degree in Social Work or related field.
• Current DC professional license as applicable i.e. LSWA.
• Three years of experience working with consumers in mental health setting.

Responsibilities

• Provide services to consumers per ACT Fidelity Model.
• Lead the IRP development with the team and consumer.
• Coordinate treatment planning and discharge meetings at area hospitals.
• Educate consumers on ADL skills and health benefits.
• Update IRP, FPSA, LOCUS, GAINS, and Crisis Plan every 180 days.
• Provide an average of 4.25 hours of services per day.
• Maintain professional relationships with hospital staff.
• Ensure adherence to DMH and DC social work policies.
• Participate in treatment team meetings and follow-ups.
• Conduct needs and risk assessments.
