Job description

North London NHS Foundation Trust has a vacancy for a Mental Health Nurse (Care Co-ordinator) at HMP Pentonville, situated in North London. The core responsibility is to support the delivery of care aligned with New Models of Care.

The role involves providing high-standard mental health support within prison settings, working with healthcare staff and the wider prison establishment, particularly offering specialist advice to GPs in assessment and treatment of inmates with mental health issues.

The successful candidate will need to adopt a recovery and enablement model, facilitating the seamless transition of prisoners with mental health problems back to community-based mental health services upon release.

Applicants must be qualified RMNs with experience in mental health, forensic mental health services, and offender care. This role offers excellent development opportunities within the Health in Justice Team, as the prison services are transforming through partnerships with other healthcare providers.

Main duties of the job
  1. Work as a member of the Mental Health Service at HMP Pentonville.
  2. Operate across service functions including early days in custody, planned care, crisis care, and release/transfer.
  3. Implement clinical skills in mental health practice, including CPA care coordination, group work, risk assessment, and crisis intervention.
  4. Manage an individual caseload under the supervision of the Mental Health Hub Manager.
  5. Conduct assessments of patients’ emotional and psychological needs, interpreting complex data from multiple sources.
  6. Provide psychosocial interventions for patients with moderate to complex mental health difficulties.
  7. Facilitate group work collaboratively with multidisciplinary colleagues.
  8. Complete clinical risk assessments and document key risks accurately.
  9. Develop care plans in collaboration with patients, carers, and relevant agencies.
