This is an exciting opportunity to join a thriving Primary Care Network in North West London. As a Care Coordinator you will focus on providing personalised carebased on local needs and health priorities. Working as part of a team to deliver well-coordinatedcare, especially to vulnerable patients or those with complex health issues.
This job requirescollaboration with GP practices, the Local Authority and other organisations to ensure patients getthe best care at the time.The post-holder will work closely with practices within the PCN, requiring strong communicationand problem-solving skills, along with a deep understanding of healthcare data analysis andperformance monitoring.
The role will focus on identifying trends, improving service efficienciesand reducing health inequalities by enabling targeted interventions for key patient groups.
By collaborating with primary care colleagues and wider healthcare stakeholders, the CareCoordinator will support the network in navigating an evolving healthcare landscape, ensuringinformed decision-making and contributing to the continuous improvement of patient outcomesand service delivery.
INTERVIEWS WILL BE HELD ON THURSDAY 19TH JUNE
Coordinate care for patients across multiple services and health professionals
Liaise with patients, carers and healthcare teams to ensure effective care delivery
Assist with patient assessments, care plans and follow-up activities
Support patients with accessing community resources and health services
Manage and track referrals and appointments to ensure timely care delivery
Create and run searches using EMIS to extract and analyse relevant data.
Develop and maintain templates, protocols and concepts in EMIS to streamline practiceworkflows.
Utilise clinical tools such as Abtrace, iPlato, and Accurx to enhance digital healthcare deliveryacross practices.
Undertake additional responsibilities aligned with the role as directed by the ProgrammeDirector.
Harness Care Ltd and Harness Primary Care Networks (North and South) provides general andextended medical services to a population of 150,000 patients residing across Brent.
Harness North PCN consists of 10 GP practices based over 8 sites, caring for a population of over75,000 patients.
We work together with a range of local providers, including acute trusts, community services, socialcare, voluntary sector and patient participation groups, to offer more personalised, co-ordinatedhealth and social care to our local population.
