Job overview: An exciting opportunity has arisen for an experienced, autonomous, and highly motivated nurse to join the Memory Assessment Service, covering the South Leeds area. The role involves the assessment, diagnosis, and post-diagnostic support of people with or suspected of having Dementia. You will independently assess individuals and work with their loved ones, primarily at home, to make comprehensive judgments on cognitive impairment. Feedback will be provided to the wider MDT team for timely diagnosis and support. This position does not hold a caseload.
You will be encouraged to undertake professional development and participate in PDP, supervision, and service improvement initiatives. A car driver is essential for this post.
We are seeking Band 5 Nurses for the Leeds Memory Services, a city-wide service focused on assessment and diagnosis of dementia. This position is for the south locality. The service provides diagnostic assessments but does not include ongoing care coordination; referrals are the responsibility of the post holder if ongoing support is needed. The role requires a passion for working with people with Dementia, commitment to older adults, and active participation in service development. The post requires high clinical judgment, decision-making, communication skills, autonomous working, and effective risk assessment.
