Job summary

The Memory Assessment Practitioner role across the county is a 12-month fixed-term or secondment opportunity within Lincolnshire Partnership NHS Foundation Trust. You will assess patients, discuss dementia diagnoses and treatment options, and participate in the Memory Assessment Pathway to support timely access to care.

The role combines remote or clinic-based appointments with travel across the county. Hybrid working is available, and the service aims to broaden access to specialist memory

Qualifications

• Foundation Degree in Psychology or equivalent qualification in health and social care subject and/or qualification at Level 4 / foundation degree or above
• Awareness of Mental Health Act legislation
• Higher education in a health related subject (Desirable)
• Evidence of CPD (Desirable)

Responsibilities

• Undertake remote (via video call) or clinic-based appointments in line with the Memory Pathway and Protocols
• Make initial contact with patients and carers to explain the memory pathway
• Undertake detailed history-taking using a holistic memory assessment framework
• Record and document clinical discussions on RiO system
• Present comprehensive findings to the MDT and communicate any diagnosis to the person assessed and loved ones
• Monitor and support recommended treatment where appropriate
