Job summary

NSFT’s Memory Assessment Team in Norwich is seeking a Senior Practitioner/Memory Assessor to deliver comprehensive memory assessments and person-centred care. The role works within a multidisciplinary team across Central Norfolk, with clinic work at The Sophie Centre and some home visits as needed.

You will assess cognitive changes, contribute to accurate diagnoses, and develop holistic treatment plans while supporting students and staff through CPD and service development.

Qualifications

• Registered Nurse or Occupational Therapist with the relevant professional registration.
• Practice Supervisor / Practice Assessor training (NMC Standards 2018 or OT equivalence).
• Higher degree or postgraduate certificate in area of clinical/therapeutic practice.

Responsibilities

• Assess people with memory and cognition changes to work towards diagnosis and care plan.
• Collaborate within a multi-disciplinary team including psychiatrists, psychologists, nurses and OT.
• Provide learning environment for students; contribute to service development and CPD.
