Job summary

The Memory Assessment Service at Birmingham and Solihull Mental Health NHS Foundation Trust is seeking a Band 6 Occupational Therapist to join our multidisciplinary team, focusing on cognitive assessment and dementia care. You will deliver specialist OT assessments and contribute to diagnostic processes in a supportive setting.

You will work in clients' homes and clinic settings, participate in MDT discussions, and support care planning and referrals while assisting families through the memory

Qualifications

Must be a registered Occupational Therapist with HCPC.
Experience with cognitive assessment and dementia care is desirable.
Knowledge of the MOHO model and its application in practice.

Responsibilities

Deliver specialist OT assessments focusing on function with cognitive, physical, sensory and mental health considerations.
Analyse performance skills to inform memory assessments and help with diagnostic processes.
Conduct OT assessments to support diagnosis and perform holistic memory assessments.
Participate in a duty rota and support wider team duties.
Carry out assessments in clients' homes and in clinic settings.
Contribute to MDT discussions, care planning, and onward referrals.
Support service users and families throughout the memory assessment pathway.
