Responsibilities

• Support the delivery of Health Board prescribing and medicines optimisation priorities across GP practices.
• Work collaboratively with practice staff, community pharmacies, patients, carers, and other healthcare professionals to implement agreed initiatives.
• Contribute to the planning and delivery of medicines management education and training for pharmacy professionals, healthcare professionals, and support staff.
• Promote best practice and support workforce development to improve patient care and outcomes.
• Communicate effectively with patients, carers, colleagues, and multidisciplinary teams, adapting your approach to suit different audiences and settings.
• Present complex medicines-related information in a clear, accurate, and accessible manner.
• Support patients and carers to make informed decisions about their care and medicines.
• Act as an authorised witness for the destruction and disposal of controlled drugs, ensuring compliance with legislation, governance requirements, and organisational policies.
• Work independently with minimal supervision, taking responsibility for assigned work programmes and projects.
• Manage competing priorities effectively, meet deadlines, and use initiative to identify and resolve issues that support service objectives.
