Birmingham Community Healthcare NHS Foundation Trust is seeking an experienced pharmacist to lead medicines optimisation within primary care settings. You will perform medication reviews for complex regimens, liaise with clinicians, and ensure appropriate documentation and patient engagement across care pathways.

The role requires GPhC registration, CPD commitment, and independent prescribing where applicable.

Qualifications

• Pharmacist with GPhC registration and CPD commitment.
• Experience in medicines optimisation and medication reviews is desirable.
• Ability to liaise with clinicians and pharmacists to optimise therapy.

Responsabilités

• Undertake and record medication reviews for complex regimes.
• Record interventions and actions to optimise medicines use.
• Collaborate with community pharmacists and other service providers.
• Lead on medicines optimisation activities and audits.
• Ensure changes are documented in the clinical system and patients are contacted when needed.
