Job summary

Bridgnorth Medical Practice is seeking an experienced Clinical Pharmacist to lead medicines optimisation within a primary care team. The post holder will work with GPs, the Prescribing Lead, Pharmacy Technician, PCN pharmacists, nursing staff and admins to enhance safe, cost-effective prescribing and patient outcomes.

Qualifications

• Master's degree in Pharmacy (MPharm) or equivalent required.
• Independent Prescriber qualification desirable or willingness to undertake training.

Responsibilities

• Undertake medicines optimisation clinics and structured medication reviews for patients with long-term conditions and polypharmacy.
• Review repeat prescribing processes to ensure safe and effective prescribing.
• Support medicines reconciliation post-discharge and after outpatient appointments.
• Manage prescribing queries from patients, GPs, pharmacies, care homes and secondary care.
• Lead prescribing audits and medicines governance; support CQC compliance.
