Job summary

Responsibilities

• Support the Clinical Pharmacists and multidisciplinary team in medicines optimisation.
• Undertake medication switches and review patient records, implementing changes and communicating with patients.
• Assist medicines reconciliation and optimisation after hospital discharge and during care transitions.
• Identify prescribing risks through clinical system searches, audits and quality improvement initiatives.
