Job summary

Qualifications

Responsibilities

• Lead medicines optimisation work across the practice.
• Review prescribing trends and support safe, cost-effective prescribing.
• Provide advice to clinicians on formulary, guidelines, and best practice.
• Conduct structured medication reviews (SMRs) and repeat prescribing management.
• Identify interactions, contraindications, and adherence issues.
• Support chronic disease management and medicines-related care plans.
• Lead medicines safety audits and contribute to governance and QI processes.
• Collaborate with MDT members to ensure safe transitions of care and medicines reconciliation.
