We are looking for an experienced, dynamic, and compassionate Lead Pharmacist to join our supportive and forward-thinking Medicines Management Team in Ealing. What You’ll Be Doing

• Lead medicines management for the Virtual Ward, advising on medicines-related issues and pathways
• Provide expert medicines management and information to community teams
• Ensure safe, effective, and cost-efficient use of medicines
• Support prescribing decisions and provide advice to clinicians, patients, and carers
• Lead audits, governance, training, and reporting activities
• Develop and implement medicines policies and guidelines
• Collaborate with service leads and pharmacy leadership to deliver the work plan
• Participate in the weekend and out-of-hours pharmacy rota (additional pay applicable)
