We are looking for an experienced, dynamic, and compassionate Lead Pharmacist to join our supportive and forward-thinking Medicines Management Team in Ealing. This leadership role offers the opportunity to drive change, improve patient outcomes, and support integrated care across community services. You will lead medicines management for our Rapid Response Virtual Ward and provide pharmacy support across Ealing Community Health Services.

Who We’re Looking For

You’ll be a highly motivated, proactive team player with:

What You’ll Be Doing
  • Lead medicines management for the Virtual Ward, advising on medicines-related issues and pathways
  • Provide expert medicines management and information to community teams
  • Ensure safe, effective, and cost-efficient use of medicines
  • Support prescribing decisions and provide advice to clinicians, patients, and carers
  • Lead audits, governance, training, and reporting activities
  • Develop and implement medicines policies and guidelines
  • Collaborate with service leads and pharmacy leadership to deliver the work plan
  • Participate in the weekend and out-of-hours pharmacy rota (additional pay applicable)
