Type of Opportunity: Part Time (0.5 FTE) Work Schedule: Days
The medical social worker is responsible for the identification and assessment of psychosocial problems and will work with patients and families to achieve resolution or will provide referrals to the appropriate agencies for the service. The medical social worker shall be available for patients and their families through many aspects of care, including psychosocial problem-solving activities in admission and discharge phases, and assessment of financial needs and available resources, which contribute to a safe and appropriate discharge to the community setting.
Results of assessments and interventions are documented in the medical record. The medical social worker functions as an advocate and liaison to the multidisciplinary healthcare team and is responsible for discharge plans, which will achieve optimal patient outcomes. The medical social worker collaborates as a key member of the multidisciplinary healthcare team, including the active participation of the social worker in the team decision-making process. He/she is responsible for helping the team members understand the function and role of the social worker in the clinical setting.
The medical social worker acts as an expert resource and contact for the multidisciplinary team, including other social workers or community case managers, physicians, ancillary staff, and nursing staff on a multi-state and regional basis to help promote optimal continuity of care for the patient in discharge or transfer arrangements. Facilitates effective communication between administration, staff, physicians, other departments, patients, families, and the general public. Participates in conflict resolution as necessary and acts as an effective problem solver. Exemplifies a high standard of leadership qualities and professionalism. Mentors the staff in developing and achieving professional goals. Maintains current knowledge of information in the related field through seminars, workshops, and publications. Assures implementation of change projects/initiatives. Assures that the population's psychosocial and discharge planning are addressed and fulfilled. Participates in all policies, records, data, and reports as necessary for satisfaction of State, Federal, and JCAHO regulations. Serves as an educational resource regarding the functions of department-specific programs to both colleagues and customers. Completes MDS, RAPS, Care Plans. Performs Utilization Review. Performs other functions as required.
Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; Licensed as a masters social worker in the State of New Mexico.
