Job summary

University Hospitals Birmingham NHS Foundation Trust is seeking a Lung Cancer Clinical Support Worker to join the MDT, coordinating personalised care across the lung cancer pathway. You will support patients and carers, assess needs, and help navigate treatment options under supervision of Lung CNS.

You will manage data across Somerset Cancer Registry and communicate with primary care and external agencies to ensure timely access to investigations and treatments.

Qualifications

• Essential: Experience in a multi-disciplinary team.
• Essential: Experience in a healthcare setting and patient care.
• Essential: Accurate data entry and report/letter production using Word and Excel.
• Essential: Understanding of personalised care initiatives in cancer and ability to communicate with diverse patients and carers.

Responsibilities

• Integral member of the Lung Cancer MDT providing personalised care and coordination.
• Act as key contact for patients, carers and families to ensure a seamless patient-centered experience.
• Undertake holistic needs assessments and develop personalised care plans.
• Signpost to health, social care, financial and wellbeing services.
• Support patients in understanding diagnosis and treatment options; escalate concerns as needed.
• Coordinate and monitor progress through the cancer pathway and ensure timely access to appointments.
• Maintain records, support performance monitoring, audit and service improvement.
• Contribute to education and development through training and delegated clinical activities.
